Policy Prescriptions to Prevent Teen Pregnancy

Most teens would probably say they don’t want to become pregnant–in fact 87% of teen pregnancies in 2001 were reportedly unintended (see Figure 1). Even though there have been tens of thousands of teen pregnancies in recent years, teen births in the US have actually declined over the last 20 years, from 61.8 live births per 1,000 females aged 15-19 years in 1991 to 29.4 in 2012. This trend, which is due to factors that include teens making more informed decisions regarding their sexual health, is encouraging and suggests we can continue to make progress in preventing teen pregnancies.

First, we need to understand what makes teens more likely to get pregnant. Isabel Sawhill, Adam Thomas, and Emily Monea, in the Future of Children, outline several plausible explanations including cultural norms of increased acceptance of premarital sex and having children outside of marriage, a lack of positive alternatives to single motherhood, an attitude of fatalism, the high cost and limited availability of contraception, lack of knowledge about contraception and reproductive health, and inconsistent or incorrect use of contraception. The authors point out that these explanations generally fall into the categories of motivation, knowledge, and access.

Next, we can examine possible solutions. The Centers for Disease Control and Prevention recently released a Vital Signs brief outlining what the federal government, health care professionals, parents/caregivers, and teens can do to prevent teen pregnancy. What I like about the CDC’s suggestions is that they start where the teen is and show how adults can support teens’ healthy development. For example, professionals can encourage teens to delay sexual activity but should also encourage sexually active teens to consider the most effective methods of birth control. Parents can know where their teens are and what they are doing (isn’t there an app for that?), especially after school, and talk with their teens about sex.

Finally, are programs that promote these types of solutions worth the cost? Sawhill and colleagues, in their Future of Children article, conducted simulations of the costs and effects of policy initiatives that encouraged men to use condoms (motivation), discouraged teen sexual activity and educated participants about proper contraceptive use (knowledge), and expanding access to Medicaid-subsidized contraception (access). All three had good benefit-cost ratios, suggesting they are excellent social investments that can actually save taxpayer dollars. Who Needs Fathers?

With many children being raised by single mothers today, are fathers becoming dispensable or non-essential? Is child rearing more about the quality of parenting than who provides it?

Not necessarily. In a systematic review of published research on the effects of fathers’ absence on children, Future of Children Editor-in-Chief Sara McLanahan and colleagues found that the most rigorously designed studies find negative effects on child wellbeing. The strongest evidence relates to outcomes such as high school graduation, children’s social-emotional adjustment, and mental health in adulthood.

But an important consideration is the nature of the parents’ relationship. In the Fragile Families issue of Future of Children, Robert I. Lerman describes how the capacities and contributions of unwed fathers fall short of those of married fathers, but this varies by the kind of relationship the father has with the child’s mother. Clearly, father-child relationships are crucial, but the quality and stability of these relationships are at risk in fragile families.

Phillip A. Cowan, Carolyn Pape Cowan, and Virginia Knox consider how to tailor existing couple-relationship and father-involvement interventions, which are traditionally intended for married couples, to the needs of unwed parents in fragile families. The authors emphasize that improving the parental relationship, regardless of whether the parents live together, will in turn have a positive effect on fathers’ involvement. Ultimately, improving the parental relationship might also be among the most promising mechanisms of promoting marriage, which in turn could be an important component of reducing family poverty and improving child wellbeing in fragile families. Fathers are needed, and some fathers could use a little help.

Community environments protect against child maltreatment

On November 19, an international coalition of NGOs used World Day for Prevention of Child Abuse to host events about and bring attention to a threat faced by children all over the world. While most child abuse and neglect prevention strategies focus on parents – by educating them on parenting methods or treating underlying risk factors such as alcohol abuse – this coalition instead addresses the wider culture. This strategy holds that a supportive community can lead parents to make better parenting choices and can help them overcome challenges, whereas negative societal influence can overwhelm even well-intentioned parents.
In the latest The Future of Children volume, Preventing Child Maltreatment, one article looked at the community’s role in preventing child abuse from taking place. The authors found that social environment affects norms about appropriate child-raising behaviors and the acceptability of parents seeking external support when encountering challenges. In addition, positive interactions between neighbors increase the likelihood that parents will feel responsible for and act to protect all children in the neighborhood, whereas isolating and unfriendly neighborhoods may increase parental stress and their tendencies to neglect or mistreat their children. Formal community services can improve parents’ mental health and parenting capabilities and provide temporary relief from parental responsibilities.
The article highlights some innovative community programs that are designed to change a community’s atmosphere and norms to reduce child maltreatment. For instance, Triple-P in South Carolina has offered community-level information campaigns and parenting advice sessions through existing institutions such as child care centers and preschools. The Durham Family Initiative in North Carolina expands the availability of community services and uses outreach workers to build relationships in at-risk communities, address neighborhood needs, and build human capital through leadership and mentoring programs. Both these and other programs have shown promising results in reducing child abuse and neglect cases — suggesting that well-informed, well-equipped, and socially cohesive neighborhoods aid child wellbeing.
These programs face major challenges, however; costs can be significant, and changing behavior and investing in social networks can be difficult. In addition, more work needs to focus on which communities are most in need of such programs and most likely to benefit from them. Of course, individual factors play a major role in child maltreatment cases, so a community approach alone cannot solve problems of child abuse and neglect. Still, building up a supportive community is an important step toward protecting children.

Substance Abuse Treatment Alone Often Not Enough to Stem Child Abuse and Neglect

Evidence linking alcohol and other drug abuse with child maltreatment, particularly neglect, is strong. But does substance abuse cause maltreatment? In a recent article in The Future of Children volume Preventing Child Maltreatment, authors Mark Testa and Brenda Smith found that co-occurring risk factors such as parental depression, social isolation, homelessness, or domestic violence may be more directly responsible than substance abuse itself for maltreatment. Interventions to prevent substance abuse–related maltreatment, say the authors, must attend to the underlying direct causes of both.

Research on whether prevention programs reduce drug abuse or help parents control substance use and improve their parenting has had mixed results, at best. The evidence raises questions generally about the effectiveness of substance abuse services in preventing child maltreatment. Such services, for example, raise only marginally the rates at which parents are reunified with children who have been placed in foster care. The primary reason for the mixed findings is that almost all the parents face not only substance abuse problems but the co-occurring issues as well. To prevent recurring maltreatment and promote reunification, programs must ensure client progress in all problem areas.
At some point in the intervention process, attention must turn to the child’s permanency needs and well-being. The best evidence to date suggests that substance-abusing parents pose no greater risk to their children than do parents of other children taken into child protective custody. It may be sensible to set a six-month timetable for parents to engage in treatment and allow twelve to eighteen months for them to show sufficient progress in all identified problem areas. After that, permanency plans should be expedited to place the child with a relative caregiver or in an adoptive home.
Investing in parental recovery from substance abuse and dependence should not substitute for a comprehensive approach that addresses the multiple social and economic risks to child well-being beyond the harms associated with parental substance abuse.

Parenting in the Age of First Person Shooter Video Games

The hottest rage in my thirteen year old’s class last year was a first person shooter internet video game called Soldier Front. T’ween boys love these type of internet games because they can play with friends on-line; no need for playdates, the kids all meet in the virtual world and don guns together.

The goal of this game is to gun down soldiers in what easily passes for an abandoned building in Iraq. Points are given for missions accomplished and head shots (not photos of aspiring actors – bullets to the head which then explodes).
I hated this game and wanted it out of my house. With the newest software, I could block this site. But I don’t have the newest software on all my computers, I am not exactly sure how to use the parental controls properly, my eight year old could probably find his way around any filter, and – most significantly — there are many, many more games ready to fill in if this particular game is blocked. Soldier Front and others like it are not rated, so evaluating them without playing each and every one of them is impossible.
I weighed my options. I could get rid of the internet in the house (which would make it hard for me to work); I could create a filter that blocks out any content dealing with guns (including an important recent Supreme Court decision on the 2nd Amendment); I could move us to a remote part of Alaska and live off the land. None were good options. Attacking the media platform – in this case the computer and internet — rather than the content itself seemed misguided.
In 1964 Marshall McLuhan concluded that the content of electronic media, its “message,” is simply beside the point—that in electronic media, unlike print media, “the medium is the message.” In a recent volume of The Future of Children that I edited with Jeanne Brooks-Gunn, Children and Electronic Media, we came to a rather different conclusion. Content, it turns out, is critical to how media influence children. That is, the message is the message. Rather than focusing on the type of technology children use or even how much time children spend with media, parents, educators, and policymakers need to focus on what is being offered to children on the various media platforms.
This turns out to be difficult. At the government level, First Amendment considerations and the increasing reality that many media forms are exempt from government oversight makes broad regulation of content close to impossible. At the community and school level, educators struggle to use media in positive ways while ensuring that technology is not used to cheat or bully. At the family level, it is often easier for parents to tell their children, “one hour of media, that’s it,” than to wade through the content of the myriad media offerings, rely on inconsistent and hard-to-understand rating systems, and compete with an industry that often cares more about commercial success than children’s quality of life. Moreover, with the increasing portability of technology, the reality is that kids are often using electronic media where no adult is present, let alone their parents.
The answer is not more gadgets – filters, V-Chips, parental controls. Rather, industry needs to step up to the plate and do a better job in offering positive media content. In our recent policy brief we examined ways that non-profits and government are using media to positively influence adolescent well-being. These are not the public service announcements of the 1970’s and 80’s (“this is your brain on drugs”) that preached and missed the point. Rather, these are exciting, interactive, “Web 2.0” media campaigns that invite youth to create the content and own the message. Certainly for-profit industry can follow suit. While I am fairly confident that the government cannot ban first-person shooter games, I am sure that industry can decide it is not worth their time to post such games. Advertisers can push this by paying for ads on positive websites. Finally, parents, government and advocates can band together to put pressure on industry to clean up its act.
As for Soldier Front, I resorted to old fashioned parenting and simply banned the game with serious consequences if that rule was broken. It worked this time — no batteries (or broadband) required – but it may not the next when my son is older and I am not standing over his shoulder watching everything he does. Here’s hoping industry has listened by then.
